- Travel assignment with flexible scheduling based on facility needs.
Overseeing and coordinating all aspects of patient case management, ensuring high-quality, patient-centered careCollaborating with physicians, multidisciplinary teams, and support staff to create individualized care plansConducting ongoing patient assessments, utilization reviews, and discharge planningServing as a resource and advocate for patients and their families throughout the care continuumMaintaining accurate, thorough documentation in compliance with organizational and regulatory standardsEngaging in continuous quality improvement initiatives and professional development opportunitiesQualifications:
- Active RN license in good standing
- Proven experience in case management, preferably in an acute care or hospital setting

- Ability to work independently and effectively within a multidisciplinary team
- BLS certification required
