Job Summary:Provides clinical assessment expertise and coordination of referrals and admissions to post-acute services, with specific emphasis on the Inpatient Rehabilitation Program. Ensures all required documentation for the CMS-required preadmission screening for Inpatient Rehabilitation admissions. Meets with patients and family members prior to admission to provide information and establish expectations about the rehabilitation process.What You’ll Need:
- Graduation from an accredited School of Physical Therapy, Occupational Therapy or Speech Language Pathology and five years’ clinical experience in hospital setting with med/surg and/or critical care background.
- Current license to practice as a Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP) in the State of North Carolina and North Carolina driver’s license required.
What You’ll Do:
- Protects self, co-workers and facility by following approved policies and procedures and ensuring adherence to standards set forth by the appropriate licensing board.
- Ensures timely and effective response and coordination of referrals for post-acute placement.
- Facilitates and coordinates communication of referral process status and pertinent information to patients and patient families, rehab physicians, financial/screening, nursing staff, and referral sources. Ensures resolution to complex situations.
- Participates in the development of effective processes and information systems to facilitate effective measurement and execution of process performance.
- Communicates effectively with other team members, the Inpatient Rehabilitation care team, and referral sources.
- Completes preadmission screening evaluation, ensuring accuracy and completeness of required documentation, as required by CMS and the Rehab accrediting body (CARF).
- Meets with patients and their family members prior to admission to provide information and establish expectations on the rehabilitation process.
- Ensures current knowledge of regulatory and accreditation requirements for Inpatient Rehabilitation.
- Participates in Rehabilitation Committees, Task Forces and daily huddles, as requested.
- Provides age/developmentally appropriate patient care in accordance with Age/Developmental Specific Care Guidelines for the specific age groups served.
- Provides support to other Rehab support personnel, as needed, to ensure compliance with Rehab CMS requirements
- Completes insurance authorization process for inpatient rehab admissions.
- Ensures accurate collection of data related to referrals, admissions and insurance authorizations.
- Performs other duties as assigned.
SKILLS/QUALIFICATIONS:
- Excellent oral and written communication skills
- Organizational skills-ability to handle multiple complex situations simultaneously
- Good time management skills
- Solid and broad clinical knowledge base
- Critical thinking and problem-solving skills
- Knowledge of the care continuum, especially post acute care settings
- Knowledge of Medicare and Medicaid reimbursement/regulations
- Computer skills for proficiency in electronic documentation systems
- Must have access to personal vehicle
